If you lost your Red Cross certification cardPLEASE NOTE that having a De Anza transcript with 'Pass" or an A, B or C for a grade does not prove you earned a certification. It is possible to earn a passing De Anza College grade and not earn the certification.
IF YOU DID NOT GET YOUR CARD(S) IN THE MAIL YET: As I tell people in each class I teach, I turn in course records promptly, often well before the Red Cross deadline, but it can take weeks to even months before cards arrive and I can mail them out. Sorry, I can't give you a guesstimate as to when you will get your cards.
Once I get the certification cards from the Red Cross I will not be responsible for holding on to yours or for getting it to you if you did not get me a self-addressed, stamped envelope by the last class session.
If you forgot to get me an envelope before the end of class you can leave it at the faculty mailboxes. How to get a message to a De Anza instructor has a description of how to find them and a picture of the faculty mailbox drop box, but again it might not get to me and you might not get your card.
OR, more likely a way to get your card ... you can follow the instructions below to get a copy of the card from the Red Cross (or print a proof of certification).
IF YOU LOST YOUR CARD(S) I can't get you a new card. The College can't get you a new card.
If you have misplaced your American Red Cross certificate(s) and wish to receive a replacement, you must contact the American Red Cross Training Service Center 1-800-733-2767 or support@redcrosstraining.org
You will need to give them
the proper Red Cross title of the class you took, for example Cardiopulmonary Resuscitation and Automated External Defibrillation for Professional Rescuers and Health Care Providers NOT the De Anza class title HLTH 57E or just that you took a CPR class. First Aid might be filed under First Aid or under Lay Responder First Aid, but NOT under the De Anza title First Aid for the Community, Home, Wilderness and Disasters or HLTH 57A.
name of your instructor (Mary Donahue),
your name, including any possible versions you were using at the time,
and the start and end dates of the class.

Sometimes they have trouble finding where the class is filed.
Have them try looking up the CPR or first aid class under De Anza Biology, or De Anza College Biology.
Lifeguard and WSI classes will be under De Anza Physical Education or De Anza Physical Education/Athletics.
Do not let them make the mistake of looking it up under De Anza Red Cross Club or De Anza Cupertino Aquatics.
Sometimes you get a new person when you call who does not know how to find course records and might ask you to have me resubmit the course record, which I can't do because De Anza will be billed twice. If this happens please call back and you might have better luck and get someone more experienced who can better help you.
